A silver lining in GOP attempts to defund PPACA implementation

The Hill (CBO: Defunding healthcare law could end drug benefits) has an interesting piece suggesting that if Republicans are successful in blocking funding to implement the Patient Protection and Affordable Care Act (PPACA) it could mean the end of two programs that are popular with Republicans and Medicare recipients: the Medicare Part D drug benefit [...]

Clarifications About Carbohydrate and Insulin

My statements about carbohydrate and insulin in the previous post seem to have kicked up some dust! Some people are even suggesting I've gone low-fat! I'm going to take this opportunity to be more specific about my positions.

I do not think that post-meal insulin spikes contribute to obesity, and they may even oppose it. I'm not aware of anyone who researches metabolism for a living who thinks post-meal insulin spikes contribute to obesity, and after having looked into it, I understand why. It's not a controversial issue in my field as far as I can tell. Elevated fasting insulin is a separate issue-- that's a marker of insulin resistance. It's important not to confuse the two. Does insulin resistance contribute to obesity? I don't know, but it's hypothetically possible since insulin acts like leptin's kid brother in some ways. As far as I can tell, starch per se and post-meal insulin spikes do not lead to insulin resistance.

Torn Meniscus ? Symptoms, Treatment, Surgery, Recovery, Exercise

Torn Meniscus The knee joint has a fibrocartilaginous crescent-shaped structure that serves to distribute the weight of the body uniformly across the three bones that compose the knee joint namely the tibia (shin bone), femur (thigh bone) and the patella (knee cap). This structure is called the meniscus and aside from providing stability, it also acts as a spacer between the knee bones allowing them to move effectively without resulting to any damage. There are two menisci in each knee between the surfaces of the articular cartilage: the medial meniscus (at the inner section) and the lateral meniscus (at the outer section). These structures also act as shock absorbers during [...]

Saucony?s Celliant Suit Soothes Sore Muscles

Saucony?s Celliant Suit ?But really, it?s a recovery suit that Saucony gave me. It?s got this stuff called Celliant in it, and it reflects the photons??

This is what you might very well say to your friends as they look at you dumbstruck for lounging around in a racing-striped black unitard. They?d probably laugh less if you said ?I?m now a masked vigilante.?

Really, I get it. It seemed nuts to me too. Had Saucony not been the company whose shoes enabled me to start running again, I would?ve written this off to snake oil. But Saucony gear almost never fails for me.  I figured I owed it to them to have an open mind to their recovery suit, right?

Aaaand? the suit really works. Seriously. I?m more shocked than you are.

I?m not even willing to admit that it?s because of the space-aged material. I can?t tell if it makes me strangely warm because it?s inducing a sort of self-perpetuated light therapy, or because it?s made from polyester.  I don?t know if my muscles are getting fixed faster because of the Celliant, the compression, or something else.

But, the bottom line is, I recover faster when I wear the Saucony AMP PRO 2 recovery suit after a hard workout.

I was so unprepared to believe that I tried it twice.

Last Tuesday, I ran a sprint drill. I haven?t run sprints since I was maybe 20. I had my friend Courtney with me to help coach my form. But still, we ran 3-4 miles of intermittent sprints. It was a sure recipe for shin splints, cramped calves, aching thighs and the worst-case scenario: reigniting nerve-shattering knee pain. So, I slept in the suit and hoped for the best.

The best happened. The next day I had nothing but a tiny twinge in my right thigh. I was almost completely recovered.

I refused to believe it. I figured that I must not have worked as hard as I thought. So, on Saturday, I hit up the Run A Muck Festival, a 5k mud run with obstacles, boulders, and water. I went as hard as I could in the race (I wore Saucony Razors, by the way ? great for bouncing off rocks. Would you believe I can swim well in them?), and I did not wear the suit that night. By Sunday, my legs were stiff and wracked with pain. I didn?t want to get out of bed, much less walk around, with this kind of conditioning pain.

So I put on Saucony?s recovery suit. Usually, if you?re this beat up after a race, you?re stiff and sore at least through the next two days. But after lounging in the suit for three hours, I felt limber again and the pain subsided to a dull ache. I could literally feel the thing working. I was almost recovered by bedtime, and that has never happened to me before, at least not without a masseuse involved. The down side: It is warm, so you might need to crank the AC a little when you wear it. Also, you step in through the shoulders, so there is no fly, adding an extra step to a 3 a.m. bathroom break.

Whether you?re a casual athlete like me, or training for something that makes mortals tremble, you should at least try some of Saucony?s forthcoming Celliant-based products and see if they help you as much as they help me.

Uterine Artery Embolization-non surgical Fibroids treatment

A simple scar-less procedure that requires just an overnight stay at the hospital brings hope to women. According to experts like Dr Muley, Uterine Artery Embolization (UAE), a non-surgical treatment for fibroid is fast becoming popular the world over. Until now, the most common treatment for fibroid removal was hysterectomy.

But the surgery has an irreversible side effect. After the surgery, women had to forget about conception for the rest of their lives.During an UAE, however, the blood supply to the fibroid is reduced gradually until it is completely cut off from nourishment. Even after it has shrunk, the surrounding normal uterine muscles continue to get their requisite blood supply and can survive.

Uterine Artery Embolization treatment
Deprived of blood, nutrition and oxygen, the fibroids shrink during the next three to six months after the embolization treatment."Around 66 per cent of women who have undergone the UAE have conceived," says Dr Muley. What makes it even more popular is that single or multiple fibroid (up to 12-14 cm in size) can be effectively treated. And the chances of recurrence are almost negligible.

The advantages of UAE are several - it is done under local anesthesia, requires only a tiny incision in the skin and the recovery period is shorter than for, say, an open surgery. Besides, all the fibroid can be treated in one sitting with the patient suffering no blood loss. Recovery is quick, but in case there is excessive vaginal bleeding, all it takes is a couple of days of rest to be up on your feet again.

A Farewell Toast to Red-Wine Teeth

A couple days ago, I tried to think of a few things worse than being in a social setting, having a few glasses of wine, and ending up with wine-stained teeth. Let?s see, what did I come up with? Running face first into a bee hive, for one. Dashing out of a restaurant only to see your car being towed. Global warming? Ok, maybe not that bad.

It?s not merely the fact that your teeth have lost their pearly white hue that?s embarrassing. It?s more what having red-stained teeth symbolizes ? that you get a little too excited by the thought of wine. So much so, that you?d rather be in a foreign country stomping up and down in a vat of grapes than where you are right now. Or that you?re just a lush.

Up until now, our only solutions were to either stop after one glass (let?s be serious, now) or jet off to the bathroom with toothbrush and toothpaste in hand for a quick touch up. Since both of those options aren?t always convenient, it?s a good thing I stumbled upon a product called ?Wine Wipes,? from the makers Borracha LLC.

Wine Wipes uses a combination of salt, baking soda, hydrogen peroxide, calcium, glycerine, and orange blossom to bleach the teeth, neutralize acids, and provide a coating to protect against future gulps. I was ready to test these babies out!

I pulled them out of my bag one night, thinking that these wipes would come in very handy on a date. Picture it: the conversation is going well ? you both share a love of cats and long walks in the rain ? and he just can?t wait to meet your parents. Then you smile sweetly and watch as his eyes move from your face to your red teeth. And then watch as his face contorts, like he just caught a whiff of something foul.

Thankfully, I wasn?t on a date. I was just at dinner with my friends (read: I don?t care what they think of me. They?re kind of obligated to like me at this point). We all had a couple of glasses of red wine and I pulled out a wipe ? a small, circular, moist pad with one textured side for scrubbing, I assume. What immediately put me off was the taste ? a combination of sour and salty. A friend had tried one as well and mentioned, ecstatically, that they tasted like boiled peanuts.

Strange taste aside, they did seem to clean my teeth. When I looked at the wipe after I had rubbed it over my teeth, it was noticeably redder. And my teeth did look brighter. The wipes also come in a small, discreet case ? easy to take to the bathroom if you?re out in a public setting.

I?ll definitely be keeping Wine Wipes on hand for those occasions I find myself indulging in a few glasses of vino. That or just move to Europe and stomp grapes for a living.

Wine Wipes retail for $6.95 and can be purchased at stores nationwide and online.

Obesity and the Fluid-in, Fluid-out Therapy for Edema

I recently attended a lecture by Dr. Arya M. Sharma here at the University of Washington. Dr. Sharma is a Canadian clinician who specializes in the treatment of obesity. He gave the UW Science in Medicine lecture, which is a prestigious invited lecture.

He spent a little bit of time pointing out the fallacy behind conventional obesity treatment. He used the analogy of edema, which is an abnormal accumulation of fluid in the body.

Since we know that the amount of fluid contained in the body depends on the amount of fluid entering the body and the amount of fluid leaving the body, the treatment for edema is obvious: drink less, pee more.

Of course, this makes no sense. It doesn't address the underlying cause of edema and it will not help the patient. Yet we apply that exact same logic to fat loss. Since the amount of energy contained in the body (in the form of fat) depends on the amount entering and the amount leaving, the solution is easy: eat less, move more. Well, yes, if you can stick to that program it will cause fat loss. But that's equivalent to telling someone with edema to drink less water. It will cause a loss of fluid, but it won't correct the underlying problem that caused excessive fluid retention in the first place.

For example, if you have edema because your heart isn't pumping effectively (cardiac insufficiency), the heart is the problem that must be addressed. Any other treatment is purely symptomatic and is not a cure.

The same applies to obesity. If you don't correct the alteration in the system that causes an obese person to 'defend' his elevated fat mass against changes*, anything you do is symptomatic treatment and is unlikely to be very effective in the long term. My goal is to develop a method that goes beyond symptomatic treatment and allows the body to naturally return to a lower fat mass. I've been doing a lot of reading and I have a simple new idea that I feel confident in. It also neatly explains the results of a variety of weight loss diets. I've dropped a few hints here and there, but I'll be formally unveiling it in the next couple of months. Stay tuned.

* The body fat homeostasis system. The core element appears to be a negative feedback loop between body fat (via leptin, and insulin to a lesser degree) and the brain (primarily the hypothalamus, but other regions are involved). There are many other elements in the system, but that one seems to set the 'gain' on all the others and guides long-term fat mass homeostasis. The brain is the gatekeeper of both energy intake and energy expenditure, and unconscious processes strongly suggest appropriate levels for both factors according to the brain's perceived homeostatic needs. Those suggestions can be overridden consciously, but it requires a perpetual high degree of discipline, whereas someone who has been lean all her life doesn't require discipline to remain lean because her brain is suggesting behaviors that naturally defend leanness. I know what I'm saying here may seem controversial to some people reading this, because it's contrary to what they've read on the internet or in the popular press, but it's not particularly controversial in my field. In fact, you'll find most of this stuff in general neuroscience textbooks dating back more than 10 years (e.g., Eric Kandel and colleagues, Principles of Neuroscience).

Urinary infection take Cranberry juice regularly

urinary infection cranberry juiceIf you love cranberry juice, you are in luck ? scientists have discovered that it can fight off urinary tract infections in people. The research illuminates the basic mechanics of E. coli infections, possibly leading to new antibiotic drugs and infection-resistant materials for invasive medical devices.

Urinary infections are caused when virulent E. coli adhere to cells. The previous study by Terri Camesano, professor of chemical engineering at WPI, had shown that cranberry juice reduces its ability to attach to urinary tract cells.

The new study explains exactly how this happens."This is not a clinical study?it's a mechanical study that shows us the direct forces that can lead to infection," said Camesano.

The study revealed that more the amount of juice one drinks, the more the attachment force of the virulent E. coli weakened. The urine flow cannot generate enough force without the juice to break this attachment to the human cells.

Drug Cessation and Weight Gain

Commenter "mem", who has been practicing healthcare for 30+ years, made an interesting remark that I think is relevant to this discussion:
Recovering substance dependent people often put on lots of weight and it is not uncommon for them to become obese or morbidly obese.
This relates to the question that commenter "Gunther Gatherer" and I have been pondering in the comments: can stimulating reward pathways through non-food stimuli influence body fatness?

It's clear that smoking cigarettes, taking cocaine and certain other pleasure drugs suppress appetite and can prevent weight gain. These drugs all activate dopamine-dependent reward centers, which is why they're addictive. Cocaine in particular directly inhibits dopamine clearance from the synapse (neuron-neuron junction), increasing its availability for signaling.

Once the temperature reaches a point where homeostasis is compromised, the hypothalamus will try to lower it by the triggering the sweat mechanism. Sweating is the body?s inherent method of regulating temperature that is why people would sweat more if the temperature outside is high, when doing exercises or when responding to circumstances that would make them afraid, embarrassed or angry. The amount of sweat produced would depend on the number of sweat glands a person has. Normally, a person has around 2 ? 4 million [...]

Acute and Chronic Pyelonephritis ? Symptoms and Treatment

Pyelonephritis The kidneys are bean-shaped structures in the abdominal cavity that are roughly the size of a human fist. They are one of the most important excretory organs of the body. These renal structures are located at the paravertebral gutter, lying at a retriperitoneal position. The basic unit of this excretory organ is the nephrons ? responsible for filtering blood and removing the body?s waste products. These renal structures are complex reprocessing structures that filter approximately 190 liters of blood and process 2 liters of metabolic wastes and excess water per day. The surplus water and wastes would become urine that is stored in the bladder until released through urination. [...]

Health Care Reform on Judicial Fast Track

The wheels of justice, as the clich goes, grinds slowly. Given the complexity of the Patient Protection and Affordable Care Act this poses a bit of a problem. Federal District Court judges are reaching contrarian conclusions concerning the constitutionality of the PPACA. Everyone knows the validity of the health care reform law will be decided [...]

Rate Regulation Grants Announced by HHS

Carriers set health insurance premiums based on several criteria. The single biggest component is the expected cost and utilization of medical services. Then there?s the need to cover overhead (such as operations, sales costs, marketing and armies of lawyers to deal with regulation) and profit (or retained earnings for non-profits). Insurers know they don?t operate [...]

Protein in urine strong indicator of prostate cancer risk

A protein in urine could be a strong indicator of prostate cancer risk, according to British scientists who say their findings could one day be developed into a quick and simple test for the disease.

Scientists from the Cancer Research UK Cambridge Research Institute and the Institute of Cancer Research (ICR) said the protein, called microseminoprotein-beta or MSMB, is found at reduced levels in men diagnosed with the disease and are also lower in men with more aggressive forms of the cancer.

new prostate cancer test"The protein is easy to detect because it is found in urine and would potentially be a very simple test to carry out on men to identify those most at risk of developing the disease," said Hayley Whitaker of the Cambridge institute, who led the study.

Fish oil good against diabetes

Researchers at the University of California, San Diego School of Medicine have discovered why eating fish may be a great idea to ward off chronic inflammation and insulin resistance.

Jerrold Olefsky and colleagues identified a key receptor on macrophages abundantly found in obese body fat. Obesity and diabetes are closely correlated. The scientists say omega-3 fatty acids activate this macrophage receptor, resulting in broad anti-inflammatory effects and improved systemic insulin sensitivity.

Obese fat tissue contains lots of these macrophages producing lots of cytokines, which cause inflammation and rising insulin resistance.Olefsky and colleagues eventually narrowed their focus to a G-protein receptor called GPR120, which is found only on pro-inflammatory macrophages in mature fat cells. When the receptor is exposed to omega-3 fatty acids, it is activated and generates a strong anti-inflammatory effect.

diabetes diet"The omega-3 fatty acids switch on the receptor, killing the inflammatory response," said Olefsky

"Our work shows how fish oils safely do this, and suggests a possible way to treating the serious problems of inflammation in obesity and in conditions like diabetes, cancer and cardiovascular disease through simple dietary supplementation.

However, it's not clear how much fish oil constitutes a safe, effective dose. If too high, it could up the risk of to increased risk of bleeding and stroke in some people.

The stunning shift toward employed physicians

I’m amazed at just how quickly physician employment has swung from small independent practices to hospital-based employment. A Simple Way to Save Your Skin

It's safe to say that SpaFinder wants the public to feel relaxed and refreshed ? this organization provides an extensive spa directory in addition to listings of wellness classes and spa resorts. But this worldwide leader in the spa industry also wants the public to be sun smart, and with this new bracelet, they?ve just made it easier.

SpaFinder launched the ?I Will Reflect? bracelet to raise awareness about melanoma, the deadliest form of skin cancer. This white bracelet becomes purple when exposed to harmful UVA/UVB sun rays. I wore it for two weeks, and I think it?s a great reminder to wear sunscreen daily. The rubber bracelet is lightweight and convenient, but with the word ?reflect? engraved in metal, it?s obvious that this isn?t just a fashion accessory. I expected the bracelet to change colors under the bright sun, but I was surprised to see it turn purple during cloudier days.

If you often forget to apply sunscreen, I encourage you to give this product a try. The ?I Will Reflect? bracelet is available on the SpaFinder Web site for 4.95 ?
but if it saves you from skin cancer, it could be priceless.
