Food Reward: a Dominant Factor in Obesity, Part VIII

Further reading

I didn't come up with the idea that excessive food reward increases calorie intake and can lead to obesity, far from it.  The idea has been floating around the scientific literature for decades.  In 1976, after conducting an interesting diet study in humans, Dr. Michel Cabanac stated that the "palatability of the diet influences the set point of the ponderostat [system that regulates body fatness]" (1).

Currently there is a growing consensus that food reward/palatability is a major contributor to obesity. This is reflected by the proliferation of review articles appearing in high-profile journals.  For the scientists in the audience who want more detail than I provide on my blog, here are some of the reviews I've read and enjoyed.  These were written by some of the leading scientists in the study of food reward and hedonics:

Palatability of food and the ponderostat.  Michel Cabanac, 1989.
Food reward, hyperphagia and obesity.  Hans-Rudolf Berthoud et al., 2011.
Reward mechanisms in obesity: new insights and future directions.  Paul J. Kenny, 2011.
Relation of obesity to consummatory and anticipatory food reward.  Eric Stice, 2009.
Hedonic and incentive signals for body weight control.  Emil Egecioglu et al., 2011.
Homeostatic and hedonic signals interact in the control of food intake.  Michael Lutter and Eric J. Nestler, 2009.
Opioids as agents of reward-related feeding: a consideration of the evidence.  Allen S. Levine and Charles J. Billington, 2004.
Central opioids and consumption of sweet tastants: when reward outweighs homeostasis.  Pawel K. Olszewski and Allen S. Levine, 2007.
Oral and postoral determinants of food reward.  Anthony Sclafani, 2004.
Reduced dopaminergic tone in hypothalamic neural circuits: expression of a "thrifty" genotype underlying the metabolic syndrome?  Hanno Pijl, 2003.

If you can read all these papers and still not believe in the food reward hypothesis... you deserve some kind of award.

A Farewell Toast to Red-Wine Teeth

A couple days ago, I tried to think of a few things worse than being in a social setting, having a few glasses of wine, and ending up with wine-stained teeth. Let?s see, what did I come up with? Running face first into a bee hive, for one. Dashing out of a restaurant only to see your car being towed. Global warming? Ok, maybe not that bad.

It?s not merely the fact that your teeth have lost their pearly white hue that?s embarrassing. It?s more what having red-stained teeth symbolizes ? that you get a little too excited by the thought of wine. So much so, that you?d rather be in a foreign country stomping up and down in a vat of grapes than where you are right now. Or that you?re just a lush.

Up until now, our only solutions were to either stop after one glass (let?s be serious, now) or jet off to the bathroom with toothbrush and toothpaste in hand for a quick touch up. Since both of those options aren?t always convenient, it?s a good thing I stumbled upon a product called ?Wine Wipes,? from the makers Borracha LLC.

Wine Wipes uses a combination of salt, baking soda, hydrogen peroxide, calcium, glycerine, and orange blossom to bleach the teeth, neutralize acids, and provide a coating to protect against future gulps. I was ready to test these babies out!

I pulled them out of my bag one night, thinking that these wipes would come in very handy on a date. Picture it: the conversation is going well ? you both share a love of cats and long walks in the rain ? and he just can?t wait to meet your parents. Then you smile sweetly and watch as his eyes move from your face to your red teeth. And then watch as his face contorts, like he just caught a whiff of something foul.

Thankfully, I wasn?t on a date. I was just at dinner with my friends (read: I don?t care what they think of me. They?re kind of obligated to like me at this point). We all had a couple of glasses of red wine and I pulled out a wipe ? a small, circular, moist pad with one textured side for scrubbing, I assume. What immediately put me off was the taste ? a combination of sour and salty. A friend had tried one as well and mentioned, ecstatically, that they tasted like boiled peanuts.

Strange taste aside, they did seem to clean my teeth. When I looked at the wipe after I had rubbed it over my teeth, it was noticeably redder. And my teeth did look brighter. The wipes also come in a small, discreet case ? easy to take to the bathroom if you?re out in a public setting.

I?ll definitely be keeping Wine Wipes on hand for those occasions I find myself indulging in a few glasses of vino. That or just move to Europe and stomp grapes for a living.

Wine Wipes retail for $6.95 and can be purchased at stores nationwide and online.

Anthony Colpo recently posted a discussion of one of my older posts on seed oils and body fat gain (1), which reminded me that I need to revisit the idea. As my knowledge of obesity and metabolism has expanded, I feel the evidence behind the hypothesis that seed oils (corn, soybean, etc.) promote obesity due to their linoleic acid (omega-6 fat) content has largely collapsed.

New test for oral cancer detection

Researchers are on the verge of developing a new test for oral cancer that could be done by using a brush to collect cells from a patient's mouth. Scientists from the University of Sheffield would develop the new method that could provide an accurate diagnosis in less than 20 minutes for lesions where there is a suspicion of oral cancer.

The new test would involve removing cells with a brush, placing them on a chip, and inserting the chip into the analyzer, leading to a result in 8-10 minutes. This will have a number of benefits including cutting waiting times and the number of visits, and also cost savings for the NHS.

new oral cancer test
The team led by Professor Martin Thornhill has begun carrying out clinical trials on patients at Charles Clifford Dental Hospital for two years to perfect the technology and make it as sensitive as possible.

If oral cancer is detected early, the prognosis for patients is excellent, with a five-year survival rate of more than 90 percent.

The nano-bio-chips are disposable and slotted like a credit card into a battery-powered analyzer. A brush-biopsy sample is placed on the card and micro fluidic circuits wash cells from the sample into the reaction chamber.

Dietary Guidelines for Americans, My Way

I just saw this on BoingBoing.  Simple but true.

This image was created by Adam Fields

The people who design government dietary guidelines are gagged by the fact that politics and business are so tightly intertwined in this country.  Their advice will never directly target the primary source of obesity and metabolic dysfunction-- industrially processed food-- because that would hurt corporate profits in one of the country's biggest economic sectors.  You can only squeeze so much profit out of a carrot, so food engineers design "value-added" ultrapalatable/rewarding foods with a larger profit margin.

We don't even have the political will to regulate food advertisements directed at defenseless children, which are systematically training them from an early age to prefer foods that are fattening and unhealthy.  This is supposedly out of a "free market" spirit, but that justification is hollow because processed food manufacturers benefit from tax loopholes and major government subsidies, including programs supporting grain production and the employment of disadvantaged citizens (see Fast Food Nation).

Weight gain during pregnancy

Putting on more weight during pregnancy does not necessarily mean that the baby will be bigger, according to the preliminary findings of an ongoing study, which is the first of its kind in Singapore and the region.

Instead, there are ethnic differences: While Malay mothers-to-be seemed to put on the most weight, Chinese mothers had the biggest babies, said Associate Professor Chong Yap Seng of National University Hospital and the Yong Loo Lin School of Medicine at the National University of Singapore, who leads the study.

pregnancy weight gain chartBut the reasons are not yet clear, he said, as research is still in a very early stage. The Growing Up In Singapore Towards Healthy Outcomes (Gusto) study seeks to understand how pregnancy and early childhood environments can affect children's risk of developing obesity and diseases like Type 2 diabetes.

For example, other studies have found that poor nutrition during pregnancy often leads to babies being born small, but those same children are more likely to get fat and suffer from metabolic illnesses later in life.

The Gusto study tracks about 1,200 mothers. But only about 500 babies have been born to them to date, with the last batch of infants due to be delivered next April.

Joint and muscle pain - take ginger

A new study has suggested that daily doses of raw or heat-treated ginger are effective for relieving muscle pain following strenuous exercise.Though a favorite remedy of Chinese medicine practitioners for centuries, ginger has not been studied widely as a pain reliever.

Some research, however, has shown that ginger may have anti-inflammatory and analgesic properties similar to non steroidal anti-inflammatory drugs. In one study, four to 36 weeks of daily ginger doses (30 to 500 mg.) achieved reductions in knee pain from osteoarthritis.

chronic muscle painResearchers from the University of Georgia and Georgia College and State University examined the efficacy of multiple days of ginger doses for relieving experimentally induced muscle pain from 18 eccentric muscle exercises.

For the study, student volunteers were tested on 11 consecutive days while taking ginger supplements. Seventy-four students were divided in three groups given either raw ginger, heated ginger or placebo.

The authors hypothesized that pain ratings after exercise would be lower in the ginger group compared to placebo subjects. Results from the subjects' responses data showed that both raw and heat-treated ginger lowered muscle pain intensity after eccentric exercise by 25 and 23 per cent, respectively.

Heat treating ginger, therefore, did not increase the analgesic benefit.

Chili peppers could reduce blood pressure

Chili peppers might just be the solution for those with high blood pressure, according to a study.
The research in hypertensive rats has shown that while the active ingredient that gives the peppers their heat?a compound known as capsaicin?might set your mouth on fire, it also leads blood vessels to relax.

"We found that long-term dietary consumption of capsaicin, one of the most abundant components in chili peppers, could reduce blood pressure in genetically hypertensive rats," said Zhiming Zhu of Third Military Medical University in Chongqing, China.

blood pressure diet foods
Those effects depend on the chronic activation of something called the transient receptor potential vanilloid 1 (TRPV1) channel found in the lining of blood vessels. Activation of the channel leads to an increase in production of nitric oxide, a gaseous molecule known to protect blood vessels against inflammation and dysfunction, Zhu explained.

The findings in rats should be confirmed in humans through epidemiological analysis, the researchers said.It isn't yet clear just how many capsaicin-containing chili peppers a day you'd have to eat to "keep the doctor away,".

Estrogen therapy increased risk of developing kidney stones

A new report has suggested that the use of estrogen therapy is associated with an increased risk of developing kidney stones in postmenopausal women. Using data from the national Women's Health Initiative study, Naim M. Maalouf, of the University of Texas Southwestern Medical Center, Dallas, examined data from two trials.

10,739 postmenopausal women with hysterectomy who received either an estrogen-only treatment or matching placebo and 16,608 postmenopausal women without hysterectomy who received either an estrogen plus progesterone treatment or matching placebo. Data were collected for an average of 7.1 years in the estrogen-only trial and 5.6 years for the estrogen plus progestin trial.

Estrogen therapy kidney stone
A total of 335 cases of kidney stones were reported in the active treatment groups, while 284 cases occurred in the placebo groups. The beginning demographic characteristics and risk factors for kidney stones were similar in the two groups.

Estrogen therapy was associated with a significant increase in risk of kidney stones. The corresponding annualized incidence rate per 10,000 women per year was 39 in the treatment group and 34 in the placebo group.

Your Daily Dose of Vitamins ? From a Spray Bottle?

In the fairytale land of health, I?m probably ?The Girl Who Cried Vitamin.? I blame all my health ?issues? (perceived and actual) on being vitamin-deficient. When I wake up tired after four hours of sleep, I blame it on a lack of iron. When my arms get fatigued while carrying bags of groceries, I wonder if that?s a warning sign that I?m low on calcium.

So I was pretty curious about the new C-Spray vitamins. According to The Marz Group, who distributes C-Spray, supplements lose between 80-90 percent of their nutrients after going through your digestive system. They say that the C-Spray is different because when you spray vitamins under your tongue as opposed to swallowing them, your body absorbs the nutrients better ? giving you more energy and protecting your immune system in the process.

With the dimensions of a travel-sized can of hairspray, C-Spray definitely packs a vitamin punch.  Each bottle of C-Spray contains the vitamin C equivalent of more than 75 oranges, but I was more surprised when I read the other levels of the nutrients present. With every serving of C-Spray, you get more than 11 nutrients, including 333 percent of your daily value of Vitamin C, 833 percent of Vitamin B12, and 250 percent of Vitamin D3.

For such a small container, C-Spray is extremely demanding. To get the full benefits of this supplement, I had to use it every four hours, spraying it six times every time, but that's not even the worst part. You?re supposed to hold the liquid under your tongue for 30 seconds before swallowing ? pure agony considering C-Spray?s sour taste couldn?t be masked by whatever sweetener was used in its ingredients.

I originally planned to test C-Spray for four weeks, but after not seeing a difference in my energy level (or taste buds), I took the passive-aggressive approach and ?forgot? to spray it during week four.

Chronic lung disease of prematurity (CLDP)

A new study has revealed that exposure to common viruses in daycare puts children with a chronic lung condition caused by premature birth at risk for serious respiratory infections.

Johns Hopkins Children Center researchers say their findings should prompt pediatricians to monitor their prematurely born patients, regardless of age, for signs of lung disease and to discuss the risks of daycare-acquired infections with the children parents.

These risks, the researchers found, include increased emergency room visits and medication use and more days with breathing problems.
chronic lung disease of prematurity
"Daycare can be a breeding ground for viruses and puts these already vulnerable children at risk for prolonged illness and serious complications from infections that are typically mild and short-lived in children with healthy lungs," said lead investigator Sharon McGrath-Morrow, M.D., M.B.A., a lung specialist at Hopkins Children's.

Investigators interviewed the parents of 111 children ages 3 and under with chronic lung disease of prematurity (CLDP) about their child's daycare attendance, infections, symptoms, emergency room visits, hospitalizations and use of medications.

Among the 22 children with CLDP who attended daycare, 37 per cent went to the ER for worsening symptoms since their last day in daycare, compared to 12 per cent of children who did not attend daycare. Deviated Septum ? Symptoms, Pictures, Treatment, Surgery

What is a Deviated Septum? The nasal septum is the bone and cartilage that divides the nasal cavities into two separate chambers. It is normally located at the midline making the nostrils symmetrical. Deviated nasal septum, therefore, is the improper alignment or displacement of the septum either to the left or right. Slight deviation does not cause any problems and does not necessitate treatment. There is even an estimate that 80% of the population have deviated nasal septum and most of them are going on with their lives without experiencing any inconveniences. It is only when there is too much deviation that certain health risks are involved. Deviated Septum Symptoms [...]

Ask Me a Question

On May 13th, I'll be recording a podcast with Chris Kresser of The Healthy Skeptic. Chris interviewed me about a year ago, and I thought it went well. Chris is a good host and asks interesting questions.

This time around, we're going to do things a bit differently. I'll start with a little overview of my current thoughts on obesity, then we'll answer reader questions. The show is going to be mostly about obesity and related matters, but I may answer a couple of questions that aren't related to obesity if they're especially interesting. There are two ways to leave questions: either in the comments section of this post, or the comments section of Chris's post. The show will air on May 24th.

Early signs of dementia

The early stages of Alzheimer's and other causes of dementia can be difficult to spot, but there are some signs that be useful in spotting the disease.

Early signs of Alzheimer's include:

Regularly misplacing everyday items, or putting things in odd places, and then forgetting or even denying having done.

Problems with everyday tasks like preparing a meal, putting it on the table, but forgetting to eat it and then preparing it all over again.

early dementia symptoms
General disorientation, such as failing to recognize familiar streets and frequently becoming confused about the time of the day.

Diminished judgment, like dressing inappropriately for the weather or being unaware of dangerous situations.

Mood or behavioral problems which may resemble depression - usually a mixture of agitation, irritability, apathy and lack of care with personal hygiene.

Early signs of vascular dementia are similar, but the decline can happen in distinct steps rather than gradually.

Ancestral Health Symposium Drama

My full Ancestral Health Symposium talk is not yet online, but reader bentlleyj74 found a link to a You Tube video of most of my exchange with Gary Taubes following the talk. You can find it here. I'm not sure who shot it, but if you'd like credit, please contact me.

I think I did an adequate job responding to his questions, but as I'm not the best at thinking on my feet, I can do better. So here are his questions and my more complete responses. This was all during one question and answer session; it was essentially an extended interrogation. I'm paraphrasing Gary's questions for the most part to make them more concise, although the sections in quotation marks are direct quotes:

Drug and device companies waking up to the lessons of the iPod

Drug companies have struggled to bring innovative new molecules to market, and that drought does not seem likely to end soon. The Affordable Care Act Still Not Affordable for Families

An Obama Administration ruling yesterday saved the federal government more than $500 billion. Detached Retina (Retinal Detachment) ? Symptoms, Causes and Surgery

Detached Retina The retina is the light-sensitive tissue found at the posterior portion of the inner eye. It focuses the images that comes through the lens and converts them into electrical impulses which are sent to the brain via the optic nerve. It contains photoreceptor cells (cones and rods) contained in its most sensitive part, the macula. The retina has around seven million cones for detailed daytime vision and color perception; and about 75 ? 150 million rods for the dim light (black and white) and peripheral visions. A light-sensitive ganglion cell helps in the reflexive responses to bright light. The retina occupies around 65 percent of the eye?s inner [...]

Obstipation

Obstipation Definition Obstipation is the failure to pass fecal matters or gas, practically a chronic constipation. It is a state where the intestines are never fully emptied and would eventually lead to obstruction. There is no definite diagnostic exam for obstipation. Identification of the condition is frequently made based on the patient?s medical history and the presenting signs and symptoms. A person who is chronically constipated would feel bloated most of the time which could lead to anorexia and abdominal discomfort or tenderness. If not treated as soon as possible, it can develop into more serious problems. If the bowel is not emptied, toxins would accumulate which could lead to [...]

The stunning shift toward employed physicians

I’m amazed at just how quickly physician employment has swung from small independent practices to hospital-based employment. Healthy Skeptic Podcast

Chris Kresser has just posted our recent interview/discussion on his blog The Healthy Skeptic.  You can listen to it on Chris's blog here.  The discussion mostly centered around body fat and food reward.  I also answered a few reader questions.  Here are some highlights:
  • How does the food reward system work? Why did it evolve?
  • Why do certain flavors we don?t initially like become appealing over time?
  • How does industrially processed food affect the food reward system?
  • What?s the most effective diet used to make rats obese in a research setting? What does this tell us about human diet and weight regulation?
  • Do we know why highly rewarding food increases the set point in some people but not in others?
  • How does the food reward theory explain the effectiveness of popular fat loss diets?
  • Does the food reward theory tell us anything about why traditional cultures are generally lean?
  • What does cooking temperature have to do with health?
  • Reader question: How does one lose fat?
  • Reader question: What do I (Stephan) eat?
  • Reader question: Why do many people gain fat with age, especially postmenopausal women?
The podcast is a sneak preview of some of the things I'll be discussing in the near future.  Enjoy!

Can You Moisturize Without the Ick Factor?

When Nivea invited Everyday Health to attend the launch of its new Hydra IQ technology, I was more than skeptical about the new product launch. As a lotion connoisseur, I?ve tried everything before ? from the glittery Disney princess lotions from my childhood to the ?imported? lotions that I filched from my grandmother?s dresser years later. Despite all my trial and error, I still haven?t committed to a moisturizer ? and I have dozens of half-used containers to prove it.

Nonetheless, I decided to go to the event because the name ?Hydra IQ? caught my eye on the invitation. I?ve heard plenty of fancy names for beauty products, but I never heard of one referring to its intelligence, so when I saw ?IQ? attached to the word ?Hydra,? I was curious. Did ?IQ? imply that these products are smarter than others? I wondered. But once I saw that Nivea was offering complimentary manicures, I was sold on attending. I figured that pretty nails would console me even if the products turned out to be a disappointment.

Nivea?s Hydra IQ technology claims to penetrate the skin?s layers for even protection and promises to hydrate the skin for more than 24 hours by maintaining the skin?s moisture level. The skin care launch includes four brand-new products, and the ingredients are also included in some of Nivea?s older skin care standbys. I tried three of its brand-new products in total, using at least one daily for two weeks.

I tend to slack off with the lotion when the weather gets warm because I hate when my skin feels greasy and sticky. But because of Nivea?s Express Hydration Freshening Gel, I don?t have that excuse anymore. It was by far my favorite Hydra IQ product because it?s made for the summer! Not only does it absorb quickly (less than two minutes), but its water-based formula makes it very lightweight. Plus, it contains mint extract, which gives your skin a cooling sensation.

I wasn?t as thrilled about Nivea?s Express Hydration Daily Lotion. Sure it kept its promise ? the lotion was fast-absorbing and it did last 24 hours ? but there are dozens of drugstore lotions that do the same thing for less than $8.99.

Out of the three Hydra IQ products I tried, Nivea?s Touch of Cashmere Body Wash was the only one I really didn?t like. The warm scent was too overpowering, and I expected it to be more moisturizing ? especially since the body wash left a slippery residue.

Nivea?s Hydra IQ technology did deliver results, but don?t purchase the entire line if you?re on a budget. The Express Hydrating Freshening Gel was the only product that was worth the fancy moniker, especially with summer heat waves and humidity levels ahead.

You can buy Nivea?s Hydra IQ products in drugstores everywhere.

Uterine Artery Embolization-non surgical Fibroids treatment

A simple scar-less procedure that requires just an overnight stay at the hospital brings hope to women. According to experts like Dr Muley, Uterine Artery Embolization (UAE), a non-surgical treatment for fibroid is fast becoming popular the world over. Until now, the most common treatment for fibroid removal was hysterectomy.

But the surgery has an irreversible side effect. After the surgery, women had to forget about conception for the rest of their lives.During an UAE, however, the blood supply to the fibroid is reduced gradually until it is completely cut off from nourishment. Even after it has shrunk, the surrounding normal uterine muscles continue to get their requisite blood supply and can survive.

Uterine Artery Embolization treatment
Deprived of blood, nutrition and oxygen, the fibroids shrink during the next three to six months after the embolization treatment."Around 66 per cent of women who have undergone the UAE have conceived," says Dr Muley. What makes it even more popular is that single or multiple fibroid (up to 12-14 cm in size) can be effectively treated. And the chances of recurrence are almost negligible.

The advantages of UAE are several - it is done under local anesthesia, requires only a tiny incision in the skin and the recovery period is shorter than for, say, an open surgery. Besides, all the fibroid can be treated in one sitting with the patient suffering no blood loss. Recovery is quick, but in case there is excessive vaginal bleeding, all it takes is a couple of days of rest to be up on your feet again.

Food Reward: a Dominant Factor in Obesity, Part VII

Now that I've explained the importance of food reward to obesity, and you're tired of reading about it, it's time to share my ideas on how to prevent and perhaps reverse fat gain. First, I want to point out that although food reward is important, it's not the only factor. Heritable factors (genetics and epigenetics), developmental factors (uterine environment, childhood diet), lifestyle factors (exercise, sleep, stress) and dietary factors besides reward also play a role. That's why I called this series "a dominant factor in obesity", rather than "the dominant factor in obesity".
